ROLE PURPOSE:

To provide a variety of generalist nursing duties, including assessment, coordination and case management of Residents and Clients. Specialist skills are required in Wound management, Medication administration, Continence and Palliative Care.
KEY RESPONSIBLITIES
Resident and Client Service:
  • Promote the Lifestyle Model of care, continuous lifestyle choice for residents within their own environment.
  • Actively promote and provide a holistic approach to care, which focuses on the individual needs and rights of the resident whilst demonstrating empathy and respect.
  • Be responsible for all aspects of direct care as delegated by the Registered Nurse. Undertake all activities in relation to resident care and other assigned duties.
  • Provide comprehensive care involving assessment, prevention, early detection, referral and health promotion.
Documentation and Administration:
  • Facilitate and directly contribute to the development of care plans, based on the outcomes of the assessment, in consultation with the resident and family/ carer and other members of the care team.
  • Develop agreed plans that assist in maintaining the independence of residents and involve the family in care giving.
  • Maintain an information system that supports and enhances the provision of accurate and reliable data and accurate documentation relating to the resident, The Laura Johnson Home systems, Resident Classification Scale and other legal and professional requirements.·
  • Resolve issues within the responsibilities of the role, which may have a detrimental effect on achieving resident outcomes.
  • Organises workload to facilitate planned care for individuals and groups.
  • Delegates to others activities commensurate with their abilities and scope of practice.
Teamwork:
  • Works collaboratively with the Registered Nurse concerning care management of residents with care needs.
  • Liaise with team members, family and other health personnel to ensure co-ordination of care for optimum resident outcomes.
Continuous Quality Improvement: Ensure all activities comply with The Laura Johnson Home policies and procedures; and relevant legislative, professional, ethical and quality standards. Participate in The Laura Johnson Home Continuous Quality Improvement (CQI) programs. Undertake both compulsory and elective training as required and demonstrate competency of practice. Occupational Health and Safety:
  • Has awareness and complies with the organisation's Occupational Health, Safety and Environmental practices.
  • Reports all workplace incidents, accidents and hazards immediately to supervisor and rectifies minor hazards that can be safely controlled.
  • Use lifting equipment, assistive devices and the personal protective equipments (PPE) as required.
  • Continued ability to meet to the Critical Job Demands for the position.
SCOPE OF AUTHORITY
  • Refers Media enquiries to the DON.
  • Expenditure Authorisation as determined by Director of Nursing
KEY SELECTION CRITERIA
1. Qualifications and Experience:
  • Current Enrolled Nurse registration with the Queensland Nursing Council is essential.
  • Qualifications relevant to Aged or Community Care and experience will be highly regarded.
  • Knowledge and skills as per ANCI competencies and demonstrated skills in clinical assessment, problem solving and decision making, with the ability to refer or seek assistance as required.

  • Demonstrated alignment with the philosophy of resident focussed care which supports dignity, independence and informed consultative decision making.
3. Interpersonal Competencies:
  • Ability to contribute to the team as demonstrated through excellent interpersonal and communication skills.

4. Achieving Competencies:
  • Demonstrated ability to evaluate and monitor quality, service and care practices and relate to Residents outcomes.
  • Proven commitment to providing on-going high levels of customer service.
5. Other:
  • Federal Police Clearance must be attained and any future charges or convictions that could potentially affect your role within The Laura Johnson Home must be disclosed.
